Despite all the hints ("Mum is still in a panic") it took me a while to work out which is Nora and which is Sylvia! I know, I should have remembered that right at the beginning of this story Milo said Sylvia is the patient they're visiting, but Nora, who must be 50 or thereabouts, still looks like a teenager -- what's her beauty secret? -- and acts even younger, whereas Sylvia seems more mature.
