As for the theme. I had the image of the girl riding on top, she tenses up in discomfort as her belly swells. Afterwards, she is exhausted and is unable to get off. A nice twist I had in mind was her failed efforts to get off only ends up arousing the player again, and she becomes panicked fearing it will trigger another growth spurt. Maybe she just manages to get off in time or failed and swells further, what do you think?

All I know from the show is she's big into cosplay and the MC makes outfits as a hobby. So including some outfits from the show would be a must, maybe he could surprise her by adjusting a favorite to fit her pregnant body. I've also seen 1 that would look quite nice without adjustments with her just complaining it's a bit tight on the chest. ;D
