Negan was going to let the doctor go but then he looked up and remembered that Eugene was watching.... he didn't want Eugene to see him in that light... being soft, so he threw the doctor in the fire specifically for Eugene to see. Even Dwight was shocked by it. He didn't kill Rosita because Olivia died for what Rosita did, that was her punishment.
