Are you using the website version of patreon? if so Log in to your Patreon account. Go to Settings, click on More and then Connected Apps from the menu bar. Then click the Connect button next to Discord under App Integrations.
Acrually somehow the option to link to connect both on Patreon isnt there (on my phone at least) unless you find Patreon's specific support page "Getting Discord Access", and hit the "connected apps" link down in the instructions. I origionally looked everywhere on both the Patreon app, website, and Discord's app, but there was literally no mention anywhere (aside from the already connected apps on Discord, which didnt list Patreon), even when trying to follow the instructions on Patreon's support page. https://support.patreon.com/hc/en-us/articles/212052266-Getting-Discord-access#h_01FEHWFEG74VEG6NV146DE93ST
