And further blackmailed into doing various other disturbing things that exacerbate his expanding condition. All while fruitlessly battling an unwanted addiction to the aforementioned janitor's...impressive portfolio. “Everything looks good,” Dr. Porter said. “No harm done to you or the babies, at least not physically. Derek, have you considered speaking to a counselor?”
“What? No. I’m fine,” said Derek as he threw a distracted glance at his phone.
“Right,” said the physician. “Well, if you change your mind, I’m happy to write you a referral —”
“Great, so we’re finished here.” Derek braced himself to struggle off the bed.
“There’s just one more thing I wanted to discuss with you,” Dr. Porter cut him off. “I’m not sure how this is possible, but I checked twice, and the latest sonogram revealed a…fourth baby. It didn’t appear in any of the previous scans. I’m not sure how we could have missed something like this. My only explanation is that the previous images were skewed somehow.”
“What!?” Derek’s mind was reeling. A fourth baby? Three had already been too many. Three had been impossibly overwhelming, and now there were four. Derek gripped the sides of the bed and swallowed hard, not looking at his churning midsection.
“I’d like to also speak to you about your diet. I know previously there was the issue of you being underweight, but now…well, most of the calories you consume are going to the fetuses at this point. And they are a bit large for their stage. I want you to cut back on fattening foods. Too much weight gain can lead to a difficult labor and other complications. In the meantime, there are some specialists I would like to consult —”
“What do you mean by ‘difficult labor’?” Derek blurted out. “Aren’t you just going to remove them surgically?”
Porter’s face was carefully neutral. “A male cesarean has not successfully been accomplished to date. The anatomy is just different, and medicine isn’t there yet. However, men have been entirely capable of giving birth naturally.”
Derek’s mind wasn’t computing. “What does that even mean?” This appointment was turning into a shitshow.
“Throughout this final trimester, your body will develop a unique birth canal that will lead directly to the anal passageway.”
Derek was stunned. “You want me to squeeze it out of my…” He couldn’t say it. It was ludicrous. Insane. He thought about the indignity of pushing babies out in such a way. How the janitor had even taunted him about it. There could be no dignity. Just squealing like a woman. Like a pig.
“It’s actually quite the miracle,” Dr. Porter continued in a would-be calming tone. “You are very unique, Mr. Carter. And you have your work cut out for you. You will be the first man ever to actually give birth to multiples, and it’s going to take a lot of work. In the weeks leading up to your due date, we will discuss strategies and medications to keep your energy up and your body stable during labor. We’ll also discuss preparations to stretch and loosen you up. In the meantime, we want to keep the babies at a manageable size. I understand you have a lot on your plate right now, so we can…”
Derek had heard enough. As if he hadn’t been traumatized enough in recent days. With effort, he heaved himself off the bed and waddled right out of the exam room while Porter was still talking.

The attack made the local news. Derek was under even more scrutiny than before, reporters lurking outside of his house, constantly photographing him and shouting questions.
Derek tried to remain poised but he felt truly rattled. He had to acknowledge that he was more fragile than he had ever been in his life, and practically defenseless in this state. The anxiety got to him.
He hated being alone now, but simultaneously hated having people around him, to see him. He was eating mechanically and continuously, relentlessly stuffing food into his mouth at any given moment.
He stopped going in to work early. He would only arrive at the office on time or even late, when the place was crowded. He stopped using the parking garage altogether, and would spend distressing amounts of time searching for street parking.
He knew he was being crazy but he couldn’t get past his fear. He couldn’t stop thinking about that man, and his knife, and what he intended to do with it.
Between his self-consciousness, and his newfound dread of being alone, Derek found that being around the janitor, disturbingly enough, pacified that anxiety a little.
“Harder,” Derek grunted out that night is the janitor fucked him into his mattress. Derek just wanted to be fucked hard enough to forget everything else.
On all fours, his round belly was consistently prodding against the bed. Another few inches, and it would be pressing flush against it. He couldn’t believe how big he was. He wasn’t even at term, not nearly. “Nngghhh...” Derek's body rocked as the janitor held his flanks, pumping hard and fast.
Derek’s tits were growing. They were jumping in size, just like the janitor had predicted. They had to be full C’s by then. The mounds were bloated and warm, bobbing about with the movements of Derek’s body. But as with a lot of the other uncomfortable details of his condition, Derek tried not to think about it. Even with his nipples hard and aching, swollen and sticking out, just looking obscene. Even when he bathed and tried not to touch them. The sensitivity was just insane. Derek could practically feel the janitor smirking behind him.
Derek grunted as the janitor suddenly took hold of his breasts. He clutched them roughly as he continued to pump, Derek groaning out and falling apart.
When the two of them laid back, languid, in the aftermath, the janitor continued to idly stroke and squeeze the soft new breasts, for once, shifting some of his attention from Derek’s baby-full belly.
Derek’s face twisted in discomfort but he tolerated it.
“Look at that. Coming in nice. God, you’re blowing up. Just getting huge. How much bigger are you gonna get for me?”
“Fuck off,” Derek grumbled, turning away and burying his face in a pillow. But for once he didn’t tell the janitor to actually leave. Things were different now. Derek was still recovering from the shock of the assault.
Derek didn’t wait to see if the janitor would stay or not, instead closing his eyes and forcing himself not to think about such things. He didn’t know how he fell asleep. It probably took an hour or more before the world faded to black. Soon he was groggily opening his eyes to see the sun just starting to peek in through the windows.
Derek had an interview today. A televised one. CorpaCorp’s stocks were holding steady, which indicated that the press conference had worked. So the investors were pushing him to do more media rounds despite that Derek felt sick at the very thought of having to show off his disgusting form.
He could hear noises coming from the kitchen. Derek managed to get up waddle downstairs to find the janitor at the counter preparing something. Another one of those signature shakes.
“I played with the flavor a little,” said the janitor, happily leering at Derek. “Give it a try.”
At first Derek, thought the janitor was passing him the milk gallon for some reason. Belatedly, Derek realized that the content of the gallon-sized jug was, indeed, the shake.
“You must be insane,” Derek said, grimacing as a janitor held the gallon up to Derek's lips. “I can’t drink all that.”
“Then drink as much as you can.”
“You still haven’t told me what the hell you’re putting in it. I saw those medicine bottles.”
Then, regrettably, Derek’s stomach chose that moment to grumble, and he remembered how ravenous he was. How ravenous he always was.
“It’s been fun,” said the janitor snidely. “Playing house. Spending all this time together. I mean, if you want me to leave, I’ll take the hint.”
Derek knew he was playing right into the janitor’s game when he threw one more glare at the other man, then clutched the jug, with aid. They tipped it to his lips.
To his own astonishment, he managed to chug down the whole thing.

Derek covered a belch. “Uhm, yes, I am happy to announce that I am now the official ambassador for Manternity,” he said in a breath. “Manternity is a global organization that’s focus is to raise awareness about male pregnancy and ensure that men experiencing this condition have access to prenatal healthcare as well as post-birth support.” There was a round of applause from the studio audience. Derek waited for it to die down before he continued, “This organization is critical now more than ever. We offer a safe space for men who have nowhere else to go. Men who are scared, confused, or even in denial. Men who are vulnerable to oppression and adversity. I will be working on-site with other Manternity workers later this month at the Mexican border where we will offer free consults and screenings to Mexican men who might not have access to the same level of prenatal care that we have here in the US.” Another round of applause.
It was one of those daytime television news shows. It was supposed to be friendly and casual, and would help amp up his image as a doting father-to-be. His company was pushing him to be some sort of male-pregnancy advocate. Appeal to the liberal snowflakes for once. The whole thing was nightmareish, but Derek understood the angle.
“Well that’s absolutely lovely,” said Tina Trent, todays host and interviewer. “Though it sounds like a lot to take on given your condition. You look like you could be having this baby at any moment!”
The audience cheered. Derek forced another smile. “Not quite there, Tina. Still a little ways to go.” He rubbed his belly in what he hoped seemed a loving manner.
“Oh really?” said Tina. “It couldn’t be long, now. When is your due date, again?”
“I’m going to leave that as a surprise,” Derek responded, to some good-natured heckling. It was a struggle to pretend to be relaxed and happy, especially when he was so flushed and sweaty, and just uncomfortably tight. His “breakfast beverage” had done a number on him. Derek covered his mouth against another embarrassing belch. He just hoped the camera cut away from him in time not to screen those awkward moments.
“So how has pregnant life been treating you?”
“It’s been great,” Derek lied. “I’m ecstatic. And I feel so high-energy.”
“It’s still hard to believe,” said Tina. “Senior VP of one of the top companies in the world pops up heavily pregnant out of nowhere.”
“Yes, it’s all still so new. I may be the most high-profile known case right now, but I think we’ll start seeing more men who are open with their pregnancies going forward.”
“Some men are completely horrified that it’s even a possibility,” said Tina.
Derek was one of them. “Oh yes, I can imagine.”
“Mr. Carter, would it be accurate to describe you as an outspoken conservative?”
“Sure,” said Derek. Here it comes.
“Are you saying that you did not seek out an abortion when you found out?”
“Of course not.” Derek feigned indignation. “Sure I was…overwhelmed…at first, but I love my baby. And frankly, I’m offended that anyone would suggest I would try to kill an innocent child. Every life is a blessing.”
“And would you say that you are pro-gay rights?”
Derek managed not to flinch. “Yes. Of course. I am proudly out.”
“You had some scathing words about gay marriage in the past. You have even been quoted as saying that homosexuality is ‘unnatural.’”
“People change. I changed. I think for a while, I was just in denial. And I was wrong.”
“Mr. Carter, some people are saying that the only reason you are taking this stance now is because of the pregnancy. It’s not exactly discreet.”
“Those people are wrong. I have felt this way for a while. I guess I just wasn’t too public with it, considering the reputation of my company.”
“Could you tell us who the father is?”
Which meant that he was again being implicitly referred to as “the mother.” Derek didn’t take the bait. “I’d rather keep that quiet for now. I’m trying to avoid too much overlap between my personal and professional lives, at least for the time being.” He fidgeted somewhat in his custom-made button-down, which felt tighter than ever. He was uncomfortable, seated there in that squishy chair, his huge belly perched on his lap. He rubbed it with both hands, genuinely this time. It wasn’t part of the act, it just felt so weird, the babies squirming restlessly inside him.
“Oh come now, not even a hint?”
Derek took in long, even breaths. He could hardly hear Tina anymore. He just felt out of it.
“Derek? Are you okay?” said Tina.
“I, um…” Derek mopped some sweat off his face. “I’m fine, just lots of movement. Sometimes it just gets…ohh…nrggghhhh…” He groaned as his belly heaved. Face twisting, he arched his back, shoving his belly forward. The tension was unbelievable!
Pressure and heat surged to his abdomen, his shirt growing impossibly tight against him, as his belly throbbed and heaved and pulsed. He gasped for breath, face hot and sweaty. His hands scrabbled helplessly at the taut, shuddering mound. He could feel his shirt truly stretching, diamonds of flesh steadily pushing out between the buttons of his shirt. Then finally, the episode ended. Derek slumped back in a daze as he foggily realized what had occurred.
He’d had a growth spurt on live television.
