Especially as they could likely have as prestigious position in the rest of the palace without the requirement to become a eunuch, if they have no contact with the concubines at all. Maomao's adoptive father seems quite a rarity, seeing how medical education from the West sounds to be rare even among non-eunuchs.

I agree with Iris: I always thought that the doctor story explains why the doctor is so relatively incompetent: to be a doctor in the rear palace, you have to be a eunuch. Not many top doctors would probably be willing to get that done for that non-powerful position

beyond any future plotlines, both stories told in this episode are pretty efficient world building, the quack doctor’s story is the first time we hear about how one might end up working as a eunuch in the rear palace (other than Maomao’s adoptive dad who became a eunuch specifically because he was one of the few people capable of performing a C-section), same for Pairin and the system in the brothels
