I didn't want to say this in the episode it happened because it'd have been a spoiler then, but now I can. But I think here with the guard it's implied that Ivankov knows people's true gender self. So in my headcanon at least the muscular guy from before actually wanted to be a woman and they were just too embarrased to admit it and that's why Ivan changed him to her.

There has been speculation that Oda somewhat regretted making Crocodile one of the first major opponents Luffy faced when he first entered the Grand Line. Due to the fact that Luffy beating him so early on made Crocodile look weaker than he actually is, even though he is in fact really strong. It's kind of insinuated that some of the biggest contributing factors to Luffy's victory were Luffy's luck, perseverance, and Crocodile's overconfidence and arrogance getting the better of him.

I think Level 6 is called Eternal Hell because the people held in there are just too dangerous and powerful for the other methods of torture to do much if anything. Hannyabal mentioned to Hancock that the recent additions to the prison such as Ace, Crocodile or Jinbe didn't even flinch during their 'baptism' ritual. So their punishment is like eternal boredom and nothing happening, staying forgotten.
