You can make any video about any type of engineering disaster and I'd gladly watch it. I have an obscure industrial accident for you; It was a piping failure during the Manhattan project. Some pipe fitter had a habit of wiping his filler wire down with a rag and wiped both steel and some crazy nickle alloy the cross contamination was enough to cause the pipe to fail and flood a trench with nitric acid and uranium. It cost ~$5 million in 1940's money...
