I'd say Fantasy-Kharma's missing his ear injury, however, I don't think Kharma would fantasize about himself being maimed. It's 1) new and probably not part of his personal identity as of yet, and 2) probably not something he'd fantasize about, so, actually makes total sense that he's "whole" whilst sandwiched between three studs.
