In Ontario, Canada, we totally use intramuscular injections of artificial ergonovine to stop postpartum bleeding. It has to be refrigerated, so for home births we send it in advance to put in peoples fridges. I have seen it used once, and it is an incredibly potent vasoconstrictor (used as a backup after oxytocin appears ineffective). The lady's entire leg started violently shaking for like 30 seconds but then stopped, and it worked just fine to get the bleeding down. It can't be given through an IV because it will cause the veins of the entire arm to constrict and prevent the medication from getting to the uterus where it is needed. It is useful to have different kinds of medicines available because different people will have different numbers of receptors in their bodies, so if oxytocin isn't doing the job then you try a drug that uses a different pathway. All of them cause calcium release in muscle cells, which causes muscle to contract tightly (which is what the uterus needs to do to stem the bleeding from the dinner-plate sized field of exposed blood vessels that suddenly appears when the placenta is shed). Are there better drugs for stopping postpartum bleeding? Ergonovine is more powerful than most of the others, but it also has strong side effects. The benefits of using the drug must exceed the risks, and most postpartum bleeding can be controlled by weaker drugs with fewer side effects. Still, there is a time and place to bring out the big guns!

My first - and worst - trip was 25i. It was a convluence of suboptimal factors. I was homeless, couchsurfing at the time so the environment was off, I was alone, listening to music, and I took entirely too much. I was young and knew nothing, really. I had school the next day and ended up going to school still tripping and EVERYONE knew, somebody I didn't even know came up to me and told me they were disappointed in me. I had to have an ex girlfriend exfiltrate me from the campus. She brought me back to her place and punished me by making me watch The Aristocats until the come down, which was suitably rough. I was passed out for twelve hours afterwards. The rest of my explorations were gravy, even the time some folks tried to rob me in a Howard Johnsons and after I had to flee from a perceived police dragnet - one of the times I had the pleasure of taking legitimate LSD and I felt like Fry after drinking 100 cups of coffee, I handled that situation gracefully when otherwise sober I'd have probably botched it. Fun times, glad I survived.

I'm pretty much a lifelong teetotaler, as much from lack of opportunity and AHDH new thing hard as anything else, so it was super interesting to get both the history of the drug (what a short history comparatively!) and use notes! Multiple Myeloma is now a labeled use for thalidomide (in the USA, I don't know about elsewhere)! It's also used off-label for a variety of other things, mostly autoimmune conditions. There's a prescribing program that among other things requires pregnancy testing every 2 to 4 weeks and use of 2 contraceptives from a list of methods considered effective or highly effective if there's any possibility at all that you could become pregnant. It's pretty similar to the Accutane prescribing program which I think more people probably have experience with.
