as far as I remember the stand user (the "ugly" woman) killed the "pretty" woman and then wore her skin before meeting hol horse. She works for dio too so he or the old witch shown in the beginning kinda arranged her meeting with hol horse and she fell in love with him. He did not even though she looked pretty so he left her in the last episode to save himself. I dont think hol horse knew that nena was an actual stand user working for dio but as I remember thats considered unknown.

In defense of Hermit Purple, not all stands are meant for combat, and you'll see more examples of that in later parts. Also, hermit purple knew where the barrel was cause it literally has psychic abilities.
