Sorry for being a bit late with an update. Making everything work as intended then testing and fixing it was quite bothersome this time.
For this week I've prepared 4 events.
First one and major one is possibility to choose Ame if Sanitary Inspector visits the cafe. That required new extra text for Sanitary Inspector second visit (basiclly short event on it's own). Then, if you choose Ame, that event can go two ways. First you can go NSFW route or if Ame is charming and nice looking enough, she can convince the Inspector with her 'natural' charm. All in all quite long event if you take into account the whole package.
Other than that, we have 2 Rai chat events, one sfw, and one nsfw if you will choose the 'generous' option in her chat event. And finally one Ame event, where you can find nsfw picture (remember, when you see the coffee icon in top right corner, try to right click the specific place on the screen).
Few people told me that they couldn't get gyaru couple, I've increased their spawn rate quite a bit, let me know if you will continue to have this problem.

Secret Care Cafe release notes ver 0.7.45:
- added nsfw event, possibility to use Ame when the sanitary inspector comes to the cafe
- added 3 new events (nsfw and sfw, 2 events for Rai's chat, 1 event for Ame's chat)
- increased spawn rate of Gyaru Couple by 50%
- bug fixed that showed Rai Massaged random event as SFW
- corrected few typos

So its exactly how many times he should. Previously 2 times and now 1 time. He will be available more when I will make more options for the girls. But now that you mention it, i probably should make him avaliable even if you dont have new scenes with him.

I am the same as Sederance, only seen the inspector enough times to get the Cynthia event. Since then no matter how less I clean up, turn off cleaning help he has never come back for me. I assumed there was a skill to summon or something that I was overlooking at the time.

I’ve done 1-3 on another version with Cynthia, this new update the guy showed up and I Persuaded with Ame, then he doesn’t show up

In total how many times did you seen him? It should be 3 right now: 1st warning, 2nd and 3rd you can choose who will persuade him or bad end.

I think I found a bug: The gallery thumbnail for Rai's nsfw event seems to appear on all pages of the gallery. The ones who were supposed to be there still play if you click them, but the picture and the hints are for Rai's event.
